78. Logopedes: Solucions davant la disfàgia en centres geriàtrics

Resumen

La disfàgia orofaríngia (DOF) es defineix com la dificultat de transportar el bol alimentari des de la boca fins a l’esòfag degut a alteracions estructurals o funcionals, com és el cas d’algunes demències que apareixen a la tercera edat. Constatat un baix nivell de coneixements en auxiliars de geriatria sobre l’alimentació de persones amb DOF i demència, i la manca de logopedes en centres geriàtrics, es presenten dos treballs: un d’intervenció en el qual es crea un curs de formació específica per aquests professionals, sobre què és la DOF i com abordar l’alimentació de les persones amb demència en aquests centres i, a continuació, un altre d’investigació amb l’objectiu de comprovar la utilitat i la validesa de l’anterior.

Es treballa amb una mostra d’11 auxiliars (5 com a grup control i 6 com a grup experimental, que realitza el curs de formació específica) i 10 usuaris amb demència i DOF (5 són atesos únicament per auxiliars del grup experimental i els altres 5 per auxiliars del grup control). La nostra investigació demostra l’eficàcia d’aquesta intervenció, ja que es produeix un augment del nivell de coneixements teòrico-pràctics dels auxiliars envers la DOF en usuaris amb demència, i un augment de la seguretat  i de l’eficàcia de la deglució en els usuaris.

Abstract 

Oropharyngeal dysphagia (OD) is defined as the difficulty to swallow food from the mouth to the esophagus due to structural or functional alterations, as it is the case of some types of dementia, such as those found in the elderly. Due to lack of information among geriatric nurse assistants in terms of feeding people with OD and dementia, and also, due to a lack of trained speech therapists in day care centers an intervention program is hereby presented, which offers specific training to geriatric nurse assistants on oropharyngeal dysphagia and the feeding needs of people with dementia who are institutionalized in day care centers. This training would be followed by research aimed at testing the usefulness and validity of the course. The sample consists of 11 geriatric nurse assistants (5 of them are in the control group and another 6 who participate as the experimental group taking the training course). Also, the sample counts with 10 patients with dementia and OD (5 who are only looked after by members of the experimental group while the rest are being taken care of by members in the control group). Our research concludes that results prove the specific training increases the theoretical and practical knowledge among geriatric nurse assistants from the experimental group. Moreover, there is an important increase in the safety and effectiveness of dysphagia patients with dementia when swallowing.
